Prior to submitting your organization's application, please upload copies of the following in PDF format: 1) your organization's most recent IRS Determination Letter, 2) most recent IRS Form W-9, and 3) ACH Direct Deposit details on bank's or your organization's letterhead (or a canceled check), including contact name, phone number, and email address for remittance advices.
