2025 Registration Form

Charity sign-ups will be open from Thursday, May 1, 2025 through Friday, October 17th, 2025. Please complete the form below to register your 501(c)(3) and click 'Submit' when you are done.You will receive a confirmation email within one to five business days once your application is approved.


(Please write exactly how you would like your organization's name shown on any Birdies for Charity materials including website, social media, and signage at the tournament.)
(Please Note: This will be the main point of contact for all Birdies for Charity related communications.)



Prior to submitting your organization's application, please upload copies of the following in PDF format:

  1. Organization's most recent IRS Determination Letter,
  2. IRS Form W-9 (Rev. March 2024) completed and signed by organization, and
  3. ACH Direct Deposit details on bank's or your organization's letterhead (or a canceled check), including contact name, phone number, and email address for remittance advices.

Additional Opportunity – Dominion Energy Charity Classic 5K


The Dominion Energy Charity Classic 5K will return to Hardywood Brewery – West Creek on Wednesday, September 24, 2025 at 6:00pm. 100% of race participant entry fees will go back to the Birdies for Charity organization selected by the participant during registration, plus an additional 10% bonus. The top three (3) organizations that recruit the most participants for their team will each receive a $1,000 bonus.


(By becoming a beneficiary, your organization agrees to promote the race and recruit participants to be part of your team.)


Charitable solicitation activity is subject to federal and state regulations. It is the responsibility of the fundraising organization to include any applicable charitable solicitation disclosures as may be required by law.

BY SIGNING BELOW AND CLICKING “SUBMIT,” THE ABOVE-IDENTIFIED CHARITY IS AGREEING TO THE 2025 TERMS & CONDITIONS OF CHARITY PARTICIPATION, AND ACKNOWLEDGES THAT PGA TOUR CHARITIES WILL ISSUE ONE (1) PAYMENT TO THE CHARITY NO LATER THAN DECEMBER 31, 2025.
