Before applying, please review the requirements below to ensure program eligibility.

Eligible organizations include the following:

  • Nonprofit organizations with 501(c)(3) tax-exempt status
  • Government entities (law enforcement or fire departments) with a funding request to serve the public
  • Educational institutions (K-12 public, private, charter schools, and colleges and universities)
To be considered for funding, an organization must:
  • Operate and serve citizens in North Carolina or South Carolina
  • Practice and provide services without discrimination as protected by law

IMPORTANT NOTES:

  • Please complete this form and upload all requested documents. You will not be able to save or submit a partial entry so please be prepared to complete the full application before submitting.
  • Please note that funds will be transferred via electronic payment. If your organization can accept electronic payments (ACH transfer/direct deposit), please proceed with this application.

Please provide all the documents listed below; failure to do so may result in your application being declined. Documents can be uploaded at the bottom of this section

Reminder: The PGA TOUR provides all payments electronically so your organization must have bank information on file to receive funds. Please note that you or your finance team will be contacted to verify the account information provided and this should not be considered a confirmation of payment.

Please Upload:
  • IRS Determination Letter (Click Here to see a sample IRS Determination letter; the tax exempt form will not be accepted)
  • Most recent W9 or 990
  • Completed bank form (Click Here for bank form)
  • Voided check OR bank letter on letterhead (one is required)

By registering for the Birdies for Better program, your organization agrees to receive communications from the tournament team related to your participation. These communications are essential to the administration and success of the program and include, but are not limited to:

  • Weekly updates on fundraising totals
  • Program announcements and updates
  • Important deadlines and logistical information

Participation in Birdies for Better requires acceptance of these communications. By completing registration, your organization acknowledges and agrees to this requirement.
